Here at Angell we consider the primary account holder to be the ‘owner’ of a pet. We understand that from time to time, circumstances dictate the need to transfer primary ‘ownership’ of a patient from one client to another. While the situations may be different – a pet moving to a new home, or someone other than the actual owner being the one who established the patient record at Angell – the process for transferring ownership is the same. To transfer ‘ownership’ of a pet’s Angell medical record to another person, you must complete a Change of Ownership form and submit copies of photo identification for both the person relinquishing ownership and the person accepting ownership.

- Signatures from both parties are required for your form to be considered complete.
- Please note that the Change of Ownership cannot be processed unless both accounts have a balance of zero.
